Terms of Service
The terms governing your use of clinic.ca's Ontario async document service.
1. About clinic.ca
clinic.ca is an Ontario async document service operated by Capital Z Technologies Inc. ("we", "us", "clinic.ca"). We connect Ontario patients with licensed Nurse Practitioners (CNO-registered, Extended Class) who review patient-submitted intake forms and, when clinically appropriate, issue lab requisitions, specialist referral letters, prescriptions, or medical notes. clinic.ca is a one-way document delivery service: once your document is delivered, the transaction is complete. We do not review or interpret lab results, and we do not provide ongoing clinical care or follow-up.
clinic.ca is a technology platform. We do not practice medicine. All clinical decisions are made by independent, licensed nurse practitioner exercising their own professional judgment.
2. Eligibility
To use clinic.ca, you must:
- Be at least 18 years of age (or have parental/guardian consent)
- Be physically located in Ontario at the time of your request
- Provide accurate and complete information during the intake process
- Have a valid email address for receiving your results and communications
3. Services and fees
Service fee
clinic.ca charges a flat service fee of $49 CAD per request. This fee covers the nurse practitioner's review of your intake submission and, if approved, the generation of your lab requisition, referral letter, or prescription. The service fee is charged at the time of submission.
Lab costs
Lab tests are performed by LifeLabs or Dynacare and may involve additional costs paid directly to the lab. If your nurse practitioner determines that your tests are medically indicated and you have a valid OHIP card, the base lab panel may be covered at no cost. Add-on tests and wellness screening panels are self-pay.
What the service fee does not include
- In-person nurse practitioner consultations
- Lab test costs at LifeLabs or Dynacare (paid separately at the lab)
- Prescription medication costs (paid at your pharmacy)
- Specialist consultation fees (covered under OHIP if referred)
4. How the service works
clinic.ca operates an asynchronous model:
- Step 1: You complete an intake questionnaire providing your health information, symptoms, and medical history
- Step 2: A licensed Ontario nurse practitioner reviews your submission within 24 hours during business hours
- Step 3: If medically appropriate, the nurse practitioner issues your requisition, referral, or prescription
- Step 4: Your document is delivered to you via email and/or your patient portal
The reviewing nurse practitioner has full authority to approve or reject any request based on their independent clinical judgment. Approval is not guaranteed.
5. Nurse Practitioner rejection and refunds
If the reviewing nurse practitioner determines that your request cannot be fulfilled — for example, because the clinical information is insufficient, the requested service is not appropriate for asynchronous care, or patient safety concerns exist — your request will be rejected.
In the event of a nurse practitioner rejection:
- You will receive a full refund of the $49 service fee to your original payment method
- Refunds are processed within 5-7 business days
- You will be notified of the rejection and may be directed to seek in-person care
clinic.ca does not provide refunds for approved requests that have been fulfilled (i.e., a requisition or prescription has been issued), unless there is a platform error.
6. Your responsibilities
By using clinic.ca, you agree to:
- Provide truthful, accurate, and complete information in your intake
- Not use clinic.ca for emergency or life-threatening conditions
- Not submit requests on behalf of another person without their knowledge and consent
- Not misrepresent your identity, age, province of residence, or OHIP eligibility
- Follow up with your family physician, nurse practitioner, or walk-in clinic if symptoms persist or worsen
7. Limitations of asynchronous care
You acknowledge that:
- Asynchronous telehealth has inherent limitations compared to in-person medical examination
- The reviewing nurse practitioner relies on the information you provide and cannot perform a physical examination
- clinic.ca is not a substitute for a primary care nurse practitioner, emergency department, or urgent care clinic
- Certain conditions, symptoms, or requests may not be appropriate for asynchronous care and will be redirected to in-person services
8. Privacy
Your personal health information is protected under Ontario's Personal Health Information Protection Act (PHIPA). Please review our Privacy Policy for full details on how we collect, use, store, and protect your information.
9. Limitation of liability
To the maximum extent permitted by Ontario law:
- clinic.ca's liability is limited to a refund of the $49 service fee you paid for the specific request giving rise to the claim
- clinic.ca is not liable for clinical decisions made by independent reviewing nurse practitioner
- clinic.ca is not liable for actions or omissions by LifeLabs, Dynacare, or any pharmacy
- clinic.ca is not liable for indirect, consequential, or incidental damages
10. Intellectual property
All content, design, and technology on clinic.ca is owned by Capital Z Technologies Inc. You may not reproduce, distribute, or create derivative works from any clinic.ca content without our written permission.
11. Governing law
These Terms are governed by the laws of the Province of Ontario and the federal laws of Canada applicable therein. Any dispute arising from these Terms or your use of clinic.ca shall be subject to the exclusive jurisdiction of the courts of Ontario.
12. Changes to these terms
We may update these Terms from time to time. Material changes will be communicated via email to active patients and posted on this page. Your continued use of clinic.ca after changes constitutes acceptance of the updated Terms.
