Is this OHIP-covered?
Insurance form completion is a private service and is not covered by OHIP. Many insurance companies reimburse patients for physician completion fees — check your policy.
What forms can clinic.ca complete?
Attending Physician Statements (APS), STD/LTD disability forms, critical illness claim forms, CPP-D applications, T2201, WSIB Form 8, life insurance forms, benefit claim forms, and return-to-work assessments.
How long does completion take?
Standard turnaround is 48–72 hours. Rush (24-hour) processing is available for an additional $49. Deadline-sensitive forms are prioritized.
Do I need to upload my form?
Recommended but not required. Email your form to forms@clinic.ca with your order ID. If you don't have the form, the MD will complete a standard Attending Physician Statement (APS) which most insurers accept.
Which insurers do you work with?
All major Canadian insurers including Sun Life, Manulife, Canada Life (Great-West), Desjardins, Industrial Alliance, RBC Insurance, Blue Cross, and employer self-insured plans. Government forms (WSIB, CPP, CRA) are also accepted.
Can I get a reimbursement from my insurer?
Many insurance policies reimburse physician completion fees. Check your policy or contact your insurer. We provide a detailed receipt for reimbursement purposes.
